Patients may qualify if they have one of these debilitating conditions and reside in the state of AZ:

  • Acquired immune deficiency syndrome (AIDS)
  • Human immunodeficiency virus (HIV)
  • Amyotrophic lateral sclerosis (ALS)
  • Agitation of Alzheimers
  • Crohns disease
  • Glaucoma
  • Hepatitis C
  • Post Traumatic Stress Disorder (PTSD)


If a chronic or debilitating disease or medical condition or the Treatment for a chronic and debilitating disease or medical condition causes:

  • Cachexia or wasting syndrome
  • Severe and chronic pain
  • Severe nausea
  • Seizures, including epilepsy characteristic
  • Severe or persistent muscle spasms, including those characteristic of multiple sclerosis

Patients must bring in medical records from the diagnosing physican (M.D., D.O., Naturopath, or Homeopath) dated within the last 12 months, and medical records must state: qualifying diagnosis, Physical exam findings, doctor’s assessment notes, and treatment/plan for the patient. Certifications are $75. Patient must self-file and are subject to additional ADHS fees.
